Insurance

I am currently in neetwork with Regence Blue Cross Blue Shield. If you have a different insurance, I can provide a superbill for out-of-network patients and help with reimbursement. Please reach out with any questions about my fees, or how I can help with out-or-network insurance billing.

If you would like to use your out-of-network provider benefits through your health insurance plan to cover services, I can help with your reimbursement process by providing a completed 1500 form and/or a SuperBill.

Reduced Fee 
Reduced fee services are available on a limited basis.
