Insurance

I am currently contracted with Regence Blue Cross Blue Shield, and Premera Blue Cross. If you have plans affiliated with either one I can bill them directly.

If you would like to use your out-of-network provider benefits through other health insurance plans besides Regence or Premera to cover services, I can help with your reimbursement process by providing a completed 1500 form, and/or a SuperBill.

Reduced Fee 
Reduced fee services are available on a limited basis.
