Insurance

I am currently contracted with Regence, and if you have Regence insurance I can bill them directly.

If you would like to use your out-of-network provider benefits through other health insurance policies besides Regence to cover services, I can help with your reimbursement process by providing a completed 1500 form, and/or a SuperBill.

Reduced Fee 
Reduced fee services are available on a limited basis.

 

Payment 
Credit cards, cash and checks are accepted.
